One concern that many patients have when considering dermal fillers to correct wrinkles and volume loss is, how long will the results last? And, what happens to my skin after filler goes away? Overwhelmingly, the question is, will my wrinkles be worse?

“The short answer is no,” says Dr. Miriam Hanson, board certified dermatologist and cosmetic expert in Austin, Texas. “Wrinkles do not become worse after having dermal fillers.” Dermal fillers comprise a family of injectable medications that restore volume in areas of the skin where it has been lost. Early fillers used purified collagen to restore and rejuvenate the skin.

The dermal fillers mentioned are not permanent, and breakdown in the skin over time. “Since the results are only temporary you can expect your pre-treatment wrinkles to re-appear after the effects of the fillers resolve,” explains Dr. Hanson. And since you have gotten accustomed to smoother skin, the re-appearance of your baseline wrinkles, may feel as though they’ve worsened. But in fact, dermal fillers cannot in any way make your wrinkles worse.

Some of the more popular fillers on the market are composed of hyaluronic acid. “The hyaluronic acid sugar is already present in body tissues. As a filler, this gel-like substance is injected below the skin. Here, it combines with water, swells and results in a ‘smoothing’ or ‘filling’ effect,” says Emily Johnston, certified physician assistant at Sanova Dermatology. Other early employed fillers were derived from collagen, a major part of skin and other tissues in the body. Collagen fillers are usually come from bovine or human sources. These types of fillers are temporary and are absorbed by the body over time.

Many fillers have demonstrated the ability to stimulate the body to lay down new collagen in the area where they are injected. This collagen sticks around even after the fillers have broken down and dissipated. Whether this is a property of the actual filler used, or a result of the distension of the skin tissue from the product being placed in the area, is unclear. Still these observations show, even on a microscopic level, that fillers have long term benefits for volume restoration, and do not worsen wrinkles once they wear off.
